چکیده
OBJECTIVE To study the impact of single instillation of 40 mg Mitomycin C (MMC-40) within first hour of transurethral resection (TUR), on first year recurrence of non-muscle invasive bladder cancer. METHODS In this study of two groups of patients with similar demographics and tumour profile were compared to assess first year tumour recurrence pattern. Group A received MMC-40 within 30 minutes of TUR. Group B patients only had TUR of bladder tumour. Patients' charts were reviewed for demographic profile, preoperative diagnosis and imaging used, cytological work up, tumour profile both during cystoscopy and imaging used, patients records were also reviewed for all subsequent check cystoscopies for recurrence. Any adjuvant treatments like intravesical chemo/immunotherapy etc. were also noted. The results were analysed using a commercially available statistical package, SPSS. The level of significance was < or = 0.05. RESULTS There were 29 and 46 patients in group A and B respectively. The demographic profile in terms of age, gender distribution, tumour characteristics (size, site, multiplicity) and pathological evaluation including, tumour grade and presence of carcinoma in situ were similar (p < 0.4 and p < 0.5) respectively. The first year recurrence rate in group A was 15% whereas it was 37.4% in group B (p < 0.04). CONCLUSIONS The first year recurrence rate is significantly decreased if MMC-40 is instilled following TUR. MMC-40 is safe and cost effective. Most low grade, low volume tumours would not require any further treatment if MMC-40 is given immediately following TUR.

Comparison of intravesical bacillus Calmette-Guerin and mitomycin C administration for non-muscle invasive bladder cancer: A meta-analysis and systematic review
The aim of the present meta-analysis was to compare the benefits of Bacillus Calmetter-Guerin (BCG) and mitomycin C in the treatment of patients with superficial bladder cancer. The present meta-analysis analyzed the benefits of BCG and mitomycin C in the treatment of patients with superficial bladder cancer by comparing progression-free survival (PFS) rates in patients treated with either of t...
متن کاملContinuous saline bladder irrigation after transurethral resection is a prophylactic treatment choice for non-muscle invasive bladder tumor.
BACKGROUND Instillation of chemotherapy after transurethral resection (TUR) has been considered as an optimal treatment for non-muscle invasive bladder cancer (NMIBC). However, controversy remains regarding the role of chemotherapy instillation. We therefore evaluated the usefulness of continuous saline bladder irrigation (CSBI) after TUR. PATIENTS AND METHODS Patients with intermediate risk ...
متن کاملSystematic Review and Individual Patient Data Meta-analysis of Randomized Trials Comparing a Single Immediate Instillation of Chemotherapy After Transurethral Resection with Transurethral Resection Alone in Patients with Stage pTa-pT1 Urothelial Carcinoma of the Bladder: Which Patients Benefit from the Instillation?
CONTEXT The European Association of Urology non-muscle-invasive bladder cancer (NMIBC) guidelines recommend that all low- and intermediate-risk patients receive a single immediate instillation of chemotherapy after transurethral resection of the bladder (TURB), but its use remains controversial. OBJECTIVE To identify which NMIBC patients benefit from a single immediate instillation. EVIDENC...
متن کامل[Therapeutic effect of mitomycin C in the immediate postoperative period in patients with intermediate-risk non-muscle-invasive bladder tumors].
INTRODUCTION Approximately 70-85% of transitional bladder cell carcinomas are non-muscle-invasive. After an initial surgery, around 60-90% will have a recurrence, being the highest risk period the first two years. Urothelium instability could be the main reason for recurrence in mid grade tumours, reason why a single dose of a chemotherapy after transurethral resection of the bladder (TURB) mig...
متن کاملUpdate on early instillation of chemotherapy after transurethral resection of non-muscle-invasive bladder cancer.
INTRODUCTION Non-muscle-invasive bladder cancer (NMIBC) is a highly recurrent disease. Early bladder chemotherapy instillation (EBCI) after transurethral resection (TURB) is an efficient way to diminish recurrence. However, this method is often challenged. Areas covered: There was a recent publication of a large meta-analysis with the original patient data and the largest study ever on EBCI. Bo...
